Seventh Grade Immunizations

Please make sure you start your student on the requirements for seventh grade.

State law requires the following:

Hepatitis B Series

Tetanus-Diphtheria booster (TD)

If your child is not immunized as per the State requirements for seventh grade, please do so now. Hepatitis B vaccine series can take as long as 6 months to complete. If your child has not begun these immunizations, he/she needs to start as soon as possible.

Have your physician or Health Department facility complete a new Florida Certificate of Immunization and send in the original blue certificate to Mrs. Hart in the front office.

If you are not sure your child is in compliance, please call your child’s physician for this information. If your child is in compliance, ask your physician to write a new Florida Certificate of Immunization. Students cannot be issued a 7th grade schedule if they are not in compliance.
